OUTCOME OF CADAVERIC HEART BEATING RENAL DONOR (CHBRD) AND ITS COMPARISON TO LIVING RENAL DONORS (LRD) (MASHHAD UNIVERVSITY MODEL)

Renal function is a predictor of long term graft survival. To assess the immediate diuresis in our center between CHBRD and LRD. In our center, harvesting the kidney is completely different to Western countries. Here, the CHBRD and their recipients brought to operation theater simultaneously, just like living donor. So the cold ischemia time was very short. Between January 2001 to August 2005, 112 HBR transplant and 45 LD kidney transplant was done in our centers. A questionnaire was prepared. After harvesting kidney (either cadaveric or living donors) and irrigating them with cold solution they were transplanted immediately in the neighboring room. Immediate diuresis & serum creatinine in day one & seven was evaluated. Cold ischemia time was less than 1 hour compare with other countries (> 24 hours) . Immediate diuresis in living donor was significantly greater than HBDL (P=0.000). There was a significant relation between immediate diuresis & serum creatinine in day seven between two groups (P=0.018). But there was no significant relation in serum creainine between two groups at the end of 6 months post op (P=0.38). 6 months graft survival in HBR recipients were 97.6% compared to 100% in L.D. Kidney transplant with this tactic has a reasonable outcome.
