This study was proposed to analyze the outcome of liver transplantation based on the gender of the donor and recipients. 130 consecutive liver transplantations performed in Shiraz (Southern Iran) Organ transplant Centre since 1993 till 2004 were studied with a minimum follow-up of 6 months. One-, two- and three-year patient and graft survival rates in all possible combinations of donor-recipient genders and in different donor groups were calculated using Kaplan- Meier analysis. The procedure and immunosuppressive regimen have been the same for nearly all patients. Among 130 consecutive recipients, 61% were male, and 39% were female. All operations were orthotopic and cadaver- sourced Survival was significantly better in female donors. However, there was no difference between survival rates among female or male recipients. No significant finding was seen in rates of rejection or other complications.
