Due to increase waiting time for kidney transplantation, efforts have been made to increase the number of available donors. One of these efforts is living unrelated donor transplantation. Our center is one of the 24 transplantation departments in Iran in which 2117(13.3%)kidney transplantations have been performed from June 1984 through March 2004, with>200 transplants per year within past 4 years. Almost all cases have been followed up except for 60 (2.8%) missed follow up. 65% of recipients are male and 35% are female, the mean age is 32±13.8 years, with the youngest 3 years old and the oldest 80. 15 years patient and graft survival are 76% and 51% respectively. Out of 2117 transplantations, 1722 cases (82%) had unrelated and 373 cases (17.8%) had related donors. Differences in graft survival were not significant, 54.4% vs. 45.3% in unrelated and related donors, respectively. Graft survival based on donors age shows higher rate in donors <30 (58.1%) than those >30 years (36.8%). (p=0.003).Only 20% of donors were female .Graft survival was higher in male donors than females (53.8% vs. 43.4%).(P=0.05).We had also 103 cases of second and 5 cases of third transplantations. The number of performed transplantations has increased due to governmental support of donors. Survival rate was not significantly different in LURD and LRD transplantations. Our higher 10 years survival might be due to: 1) Lesser dialysis duration before transplantation 2) Usage of younger donors 3) Usage of male donors
