ENBLOC KIDNEY TRANSPLANTATION OF A FIVE YEARS OLD CADAVER TO A 15 YEARS OLD RECIPIENT

This is a successful transplantation of both kidneys of a 5 years old cadaver kidneys to a 15 years old recipient. Both kidneys of a 5 years old brain dead boy who had a motorcycle accident were removed “Enbloc” with a segment of aorta and IVC. Since the kidneys were small, we decided to transplant them to a single recipient. The recipient was a 15 years old girl who was on hemodialysis because of lupus nephritis. The proximal end of donor's aortic segment was closed and the distal part was anastomosed to recipient’s common iliac artery. The proximal end of donor's caval segment was anastomosed to recipient’s IVC and the distal part was closed. Both kidneys were positioned in retroperitoneal space, just above the promontory, and ureters were anastomosed to ipsilateral native ureter in a spatulated, watertight, stented fashion. The hemodynamic condition of recipient during this 2 hours operation was acceptable. After 11 days, the recipient was discharged without any complication. During hospitalization, her serum creatinine decreased slowly and discharged with an ideal creatinine (0.8). Four weeks after transplantation two stents were removed. Now the patient voids normally and the quality of her life is changed dramatically. Although it is needed to follow up and compare such transplantation with classic ones, this is an introduction to use both kidneys of very young (<6 year) cadaver donors to a single recipient, especially in areas that request for kidney is more than average cadaveric donation.
