COMPARISON OF NONDIABETICS, PRE AND POST RENAL TRANSPLANT DIABETIC PATIENTS: A STUDY OF RISK FACTORS AND SURVIVAL

Posttransplant diabetes mellitus (PTDM), associated with the use of immunosuppressants, and occurs at varying rates in kidney transplant recipients.
The study was historical cohort. 241 patients that were transplanted from 1984 to 2003 were studied. Minimum time of follow up was 3 months. The immunosuppressive regimens were Neoral+ Prednisolone +/- Azathioprine or Mycophenolate mofetil (MMF). Underlying disease was DM in 37(15.4%) of patients. The incidence of PTDM was 6.8% in others.241 patients were put in 3 groups: 1=PTDM (5.8%), 2=nondiabetics (78.8%), 3=pretransplant DM (15.4%). ADPKD, serum Cr, Neoral & Azathioprine or MMF doses were similar in the groups. There were significant differences among groups 1 & 3 with group 2 in respect to age, weight & Prednisolone dose. The survival of group 3 was significantly less than group 1 & 2. The incidence of PTDM in our patients is low. There was no relation between ADPKD & PTDM. Age & weight were 2 important risk factors for PTDM. Survival of patients with pretransplant DM was significantly less than PTDM & nondiabetics.
