Early function of allograft after declamping is very important in outcome of transplant procedure; it reduces morbidity and mortality of recipient. This phenomenon depends several factors: donor condition, procurement of kidney, cold ischemic time, warm ischemic time, recipient condition and recipient circulation. We studied effect of extra luminal non specific vasodilator on local circulation of recipient that it is important factor in early function of allograft, so we used papaverine extraluminaly on iliac vessel of kidney recipient who prepared for anastomosing unrelated living kidney donor. After exposing iliac vessel and skeletonize vessel of kidney recipient we covered the vessel with gauze that contain solution of one ampoule of papaverine hydrochloride (40 mg /ml) figure (1). At lest after half an hour (that it takes for preparing allograft) the cover removed for anastomosing the vessel, in this time we compared the diameter of vessel in initiation of skeletonize with after covering with papaverine, there was increase in diameter of vessel and there wasn’t any spasm. This procedure was studied in thirty cases with age between 15-55years, and this group compared with thirty cases with nearly same condition without using papaverine (as control case). With using papaverine there was at least 20% increase in diameter of vessel and all of them had early function. There wasn’t any complication with extra luminal using papaverine in recipient including change in blood pressure and heart rate .the control had 7% delayed graft function. Use of extra luminal papaverine on vessel is useful and safe.
