PREGNANCY IN KIDNEY TRANSPLANT RECIPIENTS: SHAHEED LABBAFINEJAD MEDICAL CENTER EXPERIENCE

Our aim was to investigate graft, obstetric, and maternal outcomes in pregnant women with kidney allograft in our center.
Retrospective data of 74 pregnancies in 70 patients were reviewed and completed through phone interviews. The patients were compared with a stratified randomized control group of female kidney recipients, matched for age, date of transplantation, and Immunosupression regimen. Mean age of patients at transplantation was 26.55±4.72 years and median interval between transplantation and pregnancy was 27.5 months. Gestational period was 8 months. Live birth was the outcome in 43.2% of pregnancies, whereas, 9.5% led to still birth and 24.3% were aborted. In 11 pregnant patients within 12 months after transplantation, 7 live birth and 4 abortions were observed. Also, of 4 pregnancies within three months, 3 were successful. None of pregnancies, accompanied by acute rejection (AR) were successful. Twenty-six patients experienced at least one AR episode, of those 23 were after pregnancy (median interval 19 months). In control group, AR was seen in 23 patients (p=NS). However, the first AR episode occurred later in pregnant group (p=0.028). Chronic rejection and graft loss was seen in 24 and 18 cases of study group and in 17 and 17 cases of control group, respectively (p=0.125, p=NS). Median interval from the first pregnancy to graft loss was 66 months. One-, 3-, 5-, and 10-year graft survivals were 100%, 96.5%, 94.5%, and 77.1% in pregnant group and 93.2%, 85.7%, 81%, and 64.7% in control group, respectively (p=0.07). According to our findings pregnancy in kidney recipients seems to be safe for kidney allograft even within the first transplant year. Nonetheless, pregnancy outcome in this group of patients is not always favourable, especially when rejection occurs simultaneously.
