OUTCOME OF PATIENTS WITHOUT ANY IMMUNOSUPPRESSIVE MEDICATION AFTER FAILED RENAL ALLOGRAFT

The continuation of low-dose maintenance immunosuppressive medication in failed renal allografts that lifted in situ is controversially discussed. The increase in overall patient's mortality and morbidity should be considered in this setting. We evaluated outcomes of these patients without immunosuppressive therapy. In a cross sectional multicenteric study in Iran, 85 patients on hemodialysis with failed kidney transplantation (occurred at least 3 months after renal allograft), without any immunosuppressive drug consumption, evaluated for complication related to this medication. Mean and standard deviation (Mean ± SD) of age was 33.3 ± 13.4 in range of 12-56 years old. 12 patients (14.1%) underwent nephrectomy with the mean of 44.5 months after graft failure. 73 patients (85.9%) remained with no indication for nephrectomy or allograft failure syndrome such as non-infectious fever, hematuria, pain and tenderness in graft area until now. Mean ± SD of spended time was 46.5 ± 45.2 months in the last cases. These successful results are very interesting and reveal no benefit for maintenance immunosuppressive therapy in patients on hemodialysis with non-functional kidney. But, for achievement to more valid and reliable data, controlled clinical trials study on higher patient numbers with prospective design are recommended.
