KIDNEY RETRANSPLANTATION IN COMPARISON WITH FIRST KIDNEY TRANSPLANTATION

The aim of this study was to depict the outcome of second and third kidney allografts in comparison with first kidney allografts. Over 2150 kidney transplantations had been performed at Shaheed Labbafinejad Hospital, including 103 cases of second and 5 cases of third transplantations. Demographic characteristics and survivals of retransplanted patients were evaluated and compared with a randomly selected group of first kidney recipients, consisting of two matched cases with each retransplanted patients for age, sex, and date of transplantation. Retransplanted patients consisted of 78 males and 30 females with a mean age of 32.63±11.92 years. They had received kidneys from 91 living unrelated and 17 living related donors. Median follow-up was 27 months. One-, 2-, 3-, and 5-year graft survivals were 81.4%, 78.9%, 78.9%, and 73.7% in retransplant, and 92.9%, 91.5%, 89.8%, and 85.3% in control group, respectively (p=0.0037). Patient survival was 96%, 94.6%, 92.4%, and 87.8% in retransplant group and 93.1%, 92.4%, 90.9%, 87.4% in control group, respectively (p=0.63). Also, graft survivals were slightly lower in female retransplant patients compared to males in this group (p=0.09). No significant difference in survival rates was seen in different age groups. Dividing the patients into two groups based on panel reactive antibodies, antilymphocyte globulin administration, dialysis, donor relation, or previous kidney allograft duration, no significant difference was observed. However, recipients with immunosuppressi ve regimen of cyclosporine and prednisolone had lower graft survival rates compared to those with azathioperine or cellcept added (p=0.0001). It seems that kidney retransplantation can yield desirable outcome, albeit relatively lower graft survival in retransplant kidneys.
