EXPERIENCE WITH 144 CONSECUTIVE CADAVERIC RENAL TRANSPLANTATIONS

An active programme to expand the donor pool by procuring organs from cadaveric donors was started in March 1996. The aim of this paper is to analyze the results of renal transplantation (Tx) from cadaveric donors in Kuwait. 470 renal Tx procedures were performed locally from 384 (72.7%) living and 144 (27.2%) cadaveric donors. Recipients of cadaveric kidney grafts were 74 males and 70 females, aged 3 to 58 years, 30 recipients were children. Many patients had, beside renal failure, one or more other high risk factors. The mean cold ischaemia time was 12.6 hours (4 to 24 hours). Induction immunosuppression was with either ATG or anti-Interleukin-2 receptor antibodies. Recipients of cadaveric grafts were followed up for 3 to 101 months. Post Tx complications were in the form of: 43 instances of surgical complications, a single case of malignancy, 8 cases of serious bacterial infection, and 25 case of viral infections, and 37 episodes of biopsy proven acute rejection treated with methyl prednisolone or/and OKT-3. 13 recipients died with functioning graft at 3 weeks to 62 months, and 16 more grafts were lost at 1 day to 87 months after Tx. These results give 1 and 7 year actuarial survival rates of 95% and 91% for recipients, and 86% and 80% for grafts respectively.27.2% of renal grafts were obtained from local cadaveric donors, with a short cold ischaemia time and high rate of primary graft function. Results achieved in cadaveric renal Tx are comparable to those obtained in living donor Tx and to those reported by larger centers
