TEHNICAL MODIFICATIONS OF LAPAROSCOPIC DONOR NEPHRECTOMY ARE ASSOCIATED WITH IMPROVED GRAFT QUALITY AND TRANSPLANT OUTCOME

Laparoscopic Donor Nephrectomy (Lap Nx) has evolved into an attractive procedure for the donor, allowing for shorter hospitalization and faster recovery. The major disadvantages of Lap Nx are the difficult learning curve and the potential and the potential for increased garft complications and acute tubular necrosis (ATN). In June 2002, we modified the technique of Laparoscopic Nx to minimize these disadvantages. We reviewed our experience with 70 consecutive Lap Nx cases that included two sequential groups of patients, before and after introducing these modifications, respectively. Modifications of technique of Lap Nx allowed for a safer procedure, faster retrieval, better graft quality, and lesser ATN. We believe that the reduction of the pneumoperitoneum yielded better renal perfusion during renal mobilization and lessened the risk of ischemic insult. Early introduction of the Endocatch bag allowed for lateral traction of the kidney during vascular transsection, proper placement of the arterial clips and Endovascular GIA, and rapid retrieval.
